| Details | NAME OF INJURED: JOHN MUMBA OCCUPATION : CRUSHER ATTENDANT (MN ORE) PLACE OF ACCIDENT: MN ORE (CRUSHER) NATURE AND EXTENT OF INJURY: BACK PAIN DESCRIPTION OF ACCIDENT: On the early hours of 11-06-2020 at around 09:45 hrs. Mr John mumba of MN ore (batching) department was working .then he accidentally slipped on one of the stones and he landed on his back. We then applied first aid on his back to lessen the pain and we than rushed him to chitambo mission hospital for more medical treatment and letter on he was sent to serenje, chilonga mission and Italian hospital (st John hospital) for specialized treatment.
